Switching between admin and practitioner view

PT Direct uses unified credentials with the practitioner access. Now, if the user has only PT Direct admin access, when they log in, they will only see the admin console. However, if the user has both admin and practitioner access, they will see the practitioner view as the default when logging in, and then they can open the admin console via the "PT Direct" menu item.

On the PT Direct admin console, if the user wants to go back to the practitioner view, they can click the "Go to Physitrack" button.
